Esensia Clinic &amp; Laboratory in the study and application of exosome therapy for patients with type 2 diabetes mellitus. This collaboration aims to evaluate the potential of exosome therapy as an adjunctive treatment to help improve blood sugar control, suppress inflammatory biomarkers, and enhance patients&#8217; quality of life.</p><p>The initial research findings were presented by Dr. dr. Dengan ukuran berkisar 30 hingga 150 nanometer exosome mengandung kargo molekuler protein, lipid, RNA, dan DNA, berfungsi sebagai sinyal vital yang menentukan komunikasi kerjasama antar sel, jaringan dan organ.</p><p>Ukuran nanometer yg sangat kecil memungkinkan Exosomes menembus penyekat darah-otak. Dengan sifat ini Exosomes mampu memfasilitasi komunikasi dan memengaruhi proses biologis di dalam otak.</p><h2>Apa kerja Exosomes dalam tubuh kita?</h2><p>Kemampuan exosome untuk mengangkut kargo molekuler dan berkomunikasi dengan sel target telah menimbulkan minat besar dalam potensi terapeutik mereka untuk regenerasi sel dan jaringan. Berbagai penelitian menyoroti kapasitas mereka antara lain:</p><ol><li><p>Merangsang Proliferasi Sel: Exosome mengandung faktor pertumbuhan dan sinyal molekul yang merangsang pertumbuhan dan diferensiasi sel dalam regenerasi jaringan yang rusak.</p></li><li><p>Mengatur Respon Imun: Mereka dapat mengatur respons sistem imun terhadap cedera atau penyakit, menciptakan lingkungan yang mendukung penyembuhan tanpa memicu reaksi peradangan yang berlebihan.</p></li><li><p>Mengembalikan Fungsi Sel, Jaringan dan Organ: Exosome menunjukkan potensi dalam mempercepat penyembuhan luka, regenerasi tulang lunak, dan mengembalikan fungsi pada jaringan yang rusak seperti otot jantung dan saraf.</p></li><li><p>Perbaikan Neurologis: Pada kondisi neurologis, exosome menunjukkan potensi dalam memfasilitasi regenerasi sel-sel sistem syaraf. Hal ini menjanjikan harapan penyembuhan untuk berbagai kondisi gangguan degeneratif maupun gangguan perkembangan syaraf.</p></li></ol><h2>Tantangan dan Arah Masa Depan</h2><p>Meskipun exosome menjanjikan, masih banyak tantangan dalam translasi klinisnya, termasuk standarisasi metode isolasi, skalabilitas, dan penjaminan cara pembuatan exosomes yang baik.</p><p>Pertimbangan etika dan kerangka regulasi juga perlu diperhatikan seiring dengan kemajuan terapi berbasis exosome.</p><p>Namun, penelitian yang terus berlanjut dan kemajuan teknologi dalam isolasi exosome, modifikasi, dan sistem pengiriman menawarkan gambaran masa depan di mana memanfaatkan kekuatan nanopartikel biologis ini dapat merevolusi kedokteran regeneratif.</p><h2>Kesimpulan</h2><p>Exosome, yang dulu diabaikan, kini diakui sebagai agen yang menjanjikan dalam pencarian metode penyembuhan dan regenerasi jaringan yang lebih baik. Tubuh sehat mirip sebuah masyarakat sebagai sistem yang berfungsi melindungi dan merawat warganya. Fungsi sel punca mesenkimal dalam sistem tubuh mirip sekolah yang mendidik generasi sel-sel baru dengan fungsi tertentu agar organ dan jaringan berfungsi kembali. Fungsi ini dilakukan salah satunya menggunakan nanopartikel biologis, yaitu Exosomes-MSC (exosomes yang berasal dari sel punca mesenkimal) sebagai transportasi kargo bioaktif antar sel.</p><p>Berbagai spektrum penyakit kronis umumnya terjadi akibat kegagalan sistem tubuh dalam melindungi, merawat dan meregenerasi sel. Pada kejadian luka, tubuh yang sehat akan mengalami inflamasi atau radang akibat bekerjanya sistem imun dalam melindungi lokasi luka terhadap infeksi bakteri, dan membersihkan sel-sel yang rusak atau mati. Regenerasi sel dan jaringan dimulai setelah sel-sel imun selesai bekerja (imunomodulasi) dan inflamasi berhasil diredam. Ketika luka terjadi pada pasien diabetes mellitus kronis misalnya, imunomodulasi dan regenerasi sel mengalami kegagalan akibat inflamasi kronis. Exosomes-MSC yang berasal dari donor sehat berpotensi sebagai imunomodulator untuk meredam inflamasi dan meregenerasi sel, jaringan dan organ pasien.</p><h2>Penyakit apa saja yang potensial menjadi target terapi Exosomes-MSC?</h2><p>Potensi target terapi Exosomes-MSC bergantung pada kemampuan imunomodulasi dan regenerasi yang dimiliki oleh sel punca mesenkimal.</p><p>Pada pasien <strong>diabetes yang mengalami luka kronis</strong> Exosomes-MSC membantu meredam inflamasi kronis, mengembalikan jaringan kulit yang rusak (<em>tissue repair</em>), meregenerasi pembuluh darah (<em>angiogenesis</em>), dan mencegah terjadinya skar/bekas luka (<em>fibrosis</em>). Mekanisme regenerasi yang sama terjadi pada pasien liver fibrosis/cirrhosis. Fibrosis terjadi akibat inflamasi kronis pada liver sehingga angiogenesis dan perbaikan jaringan mengalami kegagalan. Secara kumulatif fibrosis dapat menjadi cirrhosis yang mengganggu fungsi organ liver.</p><p>Secara umum kemampuan Exosomes-MSC dalam melakukan imunomodulasi, perbaikan jaringan (tissue repair) dan regenerasi sel potensial berguna pada penyakit-penyakit yang melibatkan gangguan sistem imun dan inflamasi baik kronis maupun akut.</p><p>Pada pasien dengan cedera iskemik-reperfusi (Ischemic-Reperfusion Injury/ IRI) Exosomes-MSC meredam inflamasi akut akibat iskemia dan reperfusi, memulai angiogenesis, dan perbaikan jaringan pada organ terkait. Iskemia adalah kondisi dimana pembuluh darah kekurangan oksigen akibat pembekuan darah (blood clot) atau timbunan lemak (atherosclerosis). Reperfusi adalah reaksi yang segera terjadi setelah iskemia berupa aliran darah yang membawa oksigen ke lokasi cedera akibat kekurangan oksigen. Paradoksnya, reaksi ini menghambat penyembuhan karena reperfusi malah memperpanjang inflamasi akut. Cedera iskemik-reperfusi ini dapat terjadi di berbagai lokasi tubuh dan mengakibatkan disfungsi organ, seperti otak, jantung, ginjal dan tungkai (limbs) manusia.</p><p>Masih banyak agenda penelitian baik dasar maupun aplikatif yang perlu dilakukan agar segala potensi Exosomes-MSC dapat benar-benar bermanfaat bagi pasien. Di dunia beberapa tahun belakangan ini pusat-pusat penelitian biomedicine dan industri kesehatan di dunia sangat giat melakukan berbagai penelitian translasional untuk menguji Exosomes-MSC sebagai produk obat yang terstandar, aman dan bermanfaat. Di Indonesia geliat penelitian translasional ini juga tidak kalah giatnya. FKKMK UGM di Yogyakarta misalnya memiliki Tim Translasional, dipimpin oleh Prof. dr. Sofia Mubarika Haryana, <a href="http://M.Med.Sc">M.Med.Sc</a>, Ph.D yang sangat giat melakukan penelitian pra-klinis maupun klinis menggunakan Exosomes-MSC untuk berbagai spektrum penyakit. Badan Pengawasan Obat dan Makanan (BPOM) Indonesia juga telah menerbitkan beberapa peraturan mengenai obat berbasis sel (PERKA BPOM no 18 tahun 2020 tentang Pedoman Penilaian Obat berbasis sel; PERKA BPOM no 18 tahun 2022 tentang Cara Pembuatan Obat Berbasis Sel yang Baik). Pendekatan ini berbeda dari pengobatan konvensional dimana penyembuhan dilakukan dengan menghilangkan gejala-gejala yang diakibatkan berbagai penyakit.</p><p>Salah satu bentuk pengobatan regeneratif yang berkembang pesat adalah terapi berbasis sel punca mesenkimal (Mesenchymal Stem Cells/ MSC). Sel punca mesenkimal adalah kelompok sel pada orang dewasa (bukan embryo atau bayi) yang berpotensi mereplikasi diri dan berkembang menjadi berbagai macam jenis sel baru turunan sel punca tersebut. Target terapi ini adalah mengaktifkan sel punca yang ada dalam tubuh pasien agar menjadi sel-sel baru untuk memperbaiki organ atau jaringan yang rusak. Ada dua cara terapi berbasis sel dilakukan, yaitu transplantasi sel punca mesenkimal, dan infusi exosomes turunan sel punca mesenkimal (Exosomes-MSC). Pada transplantasi, sel punca yang berasal dari pasien (otogenik) atau dari donor (alogenik) ditransplantasi ke tubuh pasien.</p><p>Pada infusi Exosomes-MSC, yang masuk ke dalam tubuh pasien adalah exosomes, bukan sel punca. Exosomes-MSC merupakan nanopartikel ekstraselular (non-sel) yang secara alamiah dihasilkan oleh sel punca sebagai media transportasi kargo antar sel. Exosomes-MSC berisi material bioaktif seperti microRNA, mRNA, faktor-faktor pertumbuhan, sitokin anti-inflamasi dan lainnya yang berasal dari sel punca, dan memiliki manfaat seperti sel punca agar sel target mampu memperbaiki dan memperbarui diri. Exosomes-MSC yang digunakan sebagai terapi diproduksi di dalam laboratorium mengikuti standar cara pembuatan obat yang baik (CPOB berbasis sel; Peraturan BPOM no 18 tahun 2022). Sel punca mesenkimal yang umumnya berasal dari donor (alogenik) usia muda dikultur sampai cukup matang untuk menghasilkan sekresi berbagai macam biomolekul (aka. secretomes). Exosomes-MSC diperoleh melalui proses pemurnian lanjutan (isolasi dan filtrasi) untuk memperoleh nanopartikel dari hasil sekresi biomolekul sel punca.</p><h2>Apa potensi terapi Exosomes-MSC sebagai masa depan pengobatan regeneratif?</h2><p>Potensi utama Exosomes-MSC sebagai terapi terletak pada sifatnya sebagai kargo nanopartikel ekstraselular (non-sel) yang secara alamiah berasal dari sel punca mesenkimal. Exosomes-MSC sebagai nanopartikel memiliki ukuran sangat kecil dan relatif terstandar yaitu sekitar 30-150 nanometer (10^-9), terbuat dari material yang sama dengan membran sel (phospholipid bilayer), dan memiliki marker protein permukaan standar (CD9, CD63, CD81) yang sudah ditetapkan oleh beberapa asosiasi sains internasional terkait seperti International Society of Cell Therapy (ISCT) dan International Society of Extracellular Vesicles (ISEV).</p><p>Sebagai kargo yang berasal dari sel punca mesenkimal, Exosomes-MSC memiliki manfaat yang sama seperti yang diharapkan dari sel punca, yaitu regenerasi organ, jaringan atau sel yang nengalami disfungsi pada penyakit-penyakit kronis.</p><p>Sebagai kargo ekstraselular (non-sel) Exosomes-MSC dapat memberi manfaat tersebut pada pasien tanpa melalui transplantasi sel. Efek samping serius yang sering dilaporkan pada transplantasi sel punca mesenkimal adalah thromboembolism (penyumbatan pembuluh darah). Efek samping lain yang tergolong ringan adalah demam dan nyeri lokal. Infusi Exosomes-MSC tidak memiliki efek-efek samping tersebut karena sifatnya yang rendah immunogenisitas (low immunogenicity).</p><p>Sebagai kargo nanopartikel Exosomes-MSC tergolong stabil berada di lingkungan ekstraselular dan melalui pembuluh darah atau saluran lainnya bisa mencapai sel-sel target yang nengalami disfungsi. Kargo material bioaktif yang berasal dari sel punca donor terlindung di dalam lapisan membran phospholipid bilayer sehingga tidak terpengaruh oleh lingkungan mikro dalam tubuh pasien, dan tahan terhadap berbagai enzim tubuh yang dapat merusak. Ukurannya yang sangat kecil (30-150 nanometer) membuatnya mampu menembus blood-brain barrier dan mencapai target sel-sel neuron yang ada Di sistem syaraf.</p><p>Potensi manfaat penyembuhan dan sifat-sifat di atas membuat Exosomes-MSC menjanjikan sebagai masa depan pengobatan regeneratif. Beberapa tahun terakhir ini telah terjadi pergeseran tren yang semula terapi berbasis sel punca, menjadi terapi turunan sel punca atau terapi non sel yang berbasis sel punca. Dunia internasional baik ilmiah maupun industri sedang berlomba melakukan ujiklinik Exosomes-MSC pada berbagai penyakit agar menjadi produk yang matang digunakan sebagai terapi pengobatan.</p><p>Indonesia tidak ketinggalan dalam melakukan penelitian dan inovasi Di bidang pengobatan regeneratif dan sel punca. Sudah ada berbagai laboratorium swasta yang sudah memperoleh ijin operasional dan mengolah sel punca mesenkimal untuk menghasilkan secretomes atau exosomes. Beberapa diantaranya yang sudah memperoleh ijin operasional sejak tahun 2015 adalah ReGenic (Grup Kalbe), ProStem (Grup Prodia) dan Dermama Bioteknologi. Masih banyak tantangan yang dihadapi dalam usaha hilirisasi inovasi produk Exosomes-MSC. Subscribe for free to receive new posts and support my work.</p></div><form class="subscription-widget-subscribe"><input type="email" class="email-input" name="email" placeholder="Type your email&#8230;" tabindex="-1"><input type="submit" class="button primary" value="Subscribe"><div class="fake-input-wrapper"><div class="fake-input"></div><div class="fake-button"></div></div></form></div></div><p>Sebelum menjawab pertanyaan tersebut, mari kita telusuri terlebih dahulu: apa itu pengobatan regeneratif?</p><h2>Pengobatan Regeneratif: Dari Tubuh untuk Tubuh</h2><p>Berbeda dengan metode konvensional yang umumnya berfokus pada usaha menghilangkan gejala dari suatu penyakit atau penggantian bagian tubuh yang rusak, terapi regeneratif bertujuan untuk merangsang tubuh agar dapat memperbaiki dirinya sendiri.</p><p>Terapi regeneratif bekerja dengan memperbaiki jaringan atau organ yang rusak akibat usia, penyakit, atau cedera, dan bahkan cacat bawaan memiliki kemungkinan untuk &#8220;dinormalkan&#8221;.</p><p>Menariknya, Terapi ini tidak hanya fokus meredakan gejala, tetapi juga langsung menyasar &#8220;akar permasalahan&#8221; suatu penyakit. Beberapa teknologi yang digunakan dalam metode ini antara lain sel punca (stem cell), rekayasa jaringan, rekayasa genetik, dan terapi gen.</p><p>Menurut Dr. Anthony Atala, seorang ahli dalam bidang rekayasa jaringan dari Wake Forest Institute for Regenerative Medicine mengatakan bahwa,</p><p>&#8220;Janji utama dalam pengobatan regeneratif bukan hanya untuk membantu mengelola penyakit tetapi untuk benar-benar meningkatkan kehidupan atau bahkan memberikan penyembuhan&#8221;.</p><p>Salah satu contoh pengaplikasian pengobatan regeneratif yakni, Carticel &#8211;produk biologis pertama yang disetujui Food and Drug Administration (FDA) di bidang ortopedi.</p><p>Carticel menggunakan kondrosit autologous untuk pengobatan cacat tulang rawan artikular fokal. Pada konteks ini, kondrosit autologus diambil dari tulang rawan articular yang dikembangkan secara ex vivo, dan kemudian ditanamkan atau diletakkan pada bagian cedera, sehingga menghasilkan pemulihan yang baik.</p><div class="callout-block" data-callout="true"><p><em><strong>SciWord of the day!</strong></em></p><p><em><strong>Autologus</strong></em></p><p><em>Autologus artinya menggunakan sel, jaringan, atau cairan tubuh seseorang sendiri untuk pengobatan, seperti transplantasi darah atau terapi sel punca. Karena berasal dari tubuh sendiri, metode ini mengurangi risiko penolakan imun dan efek samping.</em></p><p><em>Dalam dunia medis, terapi autologus sering digunakan untuk mengobati kanker, penyakit autoimun, dan cedera jaringan.</em></p><p><em>Istilah autologus berasal dari bahasa Yunani &#8220;auto-&#8221; (sendiri) dan &#8220;logos&#8221; (studi). Konsep ini berkembang seiring kemajuan teknologi sel punca dan transplantasi, meskipun praktiknya sudah dilakukan sejak abad ke-20.</em></p></div><h3>Pengobatan Konvensional: Teruji dan Efektif</h3><p>Di sisi lain, pengobatan konvensional telah menjadi standar dalam dunia medis selama berabad-abad. Pendekatan ini mencangkup penggunaan obat-obatan farmasi, pembedahan, serta terapi fisik dan kimia untuk menangani berbagai penyakit. Keunggulan utama pengobatan konvensional adalah efektivitasnya yang telah terbukti secara klinis serta ketersediaannya di hampir semua fasilitas kesehatan.</p><p>Hanya saja, pengobatan konvensional umumnya berfokus pada pengelolaan gejala suatu penyakit tanpa benar-benar menyelesaikan akar masalahnya. Ibarat suatu pohon mengalami penyakit akibat terinfeksi virus, kita hanya memangkas cabang yang tampak sakit tanpa memberikan perlakuan untuk menghilangkan sumber infeksinya. Padahal, jika akar masalahnya tidak diselesaikan, infeksi bisa menyebar kembali dan merusak pohon secara berkala.</p><h3>Pengobatan Regeneratif vs. Konvensional: Kombinasi yang Sempurna untuk Masa pada akhirnya pengobatan regeneratif tidak bisa sepenuhnya dibandingkan atau dipisahkan dari pengobatan konvensional.</p><p>Hal ini dikarenakan, banyak prosedur dalam pengobatan regeneratif yang tetap membutuhkan intervensi bedah, baik untuk dapat mengimplementasikan jaringan baru maupun mengakses area yang membutuhkan perbaikan.</p><p>Sehingga, pengobatan konvensional seperti pembedahan tetap menjadi elemen penting dalam memastikan keberhasilan terapi regeneratif. Adapun mekanisme tersebut yakni, eksosom akan bekerja untuk melindungi dan memperbaiki sel pankreas, khususnya sel &#946; pankreas, yang bertugas memproduksi insulin. Kerusakan atau kematian sel &#946; pankreas merupakan salah satu penyebab utama diabetes, karena tanpa sel ini, tubuh tidak dapat menghasilkan insulin dalam jumlah yang cukup</p><p>Eksosom nantinya akan membawa &#8216;paket&#8217; berupa molekul-molekul regeneratif atau molekul yang berinteraksi untuk menghambat protein tertentu, seperti p38 untuk mencegah terjadinya kematian sel &#946; pankreas (apoptosis) dan mempercepat perbaikannya, sehingga pankreas tetap bisa menghasilkan insulin dengan normal. In 2021, the International Diabetes Federation (IDF) reported 527 million adults aged 20&#8211;79 suffering from diabetes, meaning that 1 in 10 people has diabetes, a figure that has grown by 16% (about 74 million people)since the IDF&#8217;s previous estimate in 2019. The number is projected to rise to 643 million by 2030 and 784 million by 2045.</p><p>The modern lifestyle, filled with high-sugar diets and a lack of physical activity or healthy habits, puts millions at risk of developing diabetes without them realizing it&#8212;especially those who are overweight or obese. Diabetes occurs when the body is unable to use insulin properly or does not produce enough insulin, causing blood sugar levels to spike. . If insulin is not produced normally, it disrupts blood sugar circulation. Those with type 1 diabetes rely on artificial insulin because their bodies cannot produce it at all. Some people with type 2 diabetes may also need it if their bodies have trouble using insulin effectively. This hope comes in the form of therapy using exosomes&#8212;tiny particles with remarkable potential in combating diabetes.</p><h3>Exosomes: A New Hope</h3><p>Exosomes are tiny particles produced by nearly all cells in the body, including stem cells. They contain RNA, microRNA, proteins, and lipids that can alter the function of target cells, either by repairing damaged cells or optimizing bodily functions.</p><p>Essentially, exosomes act as &#8220;couriers&#8221; or &#8220;messages&#8221; between cells, enabling communication that helps target cells heal and function more efficiently. they can assist diabetes patients in various ways through what is known as &#8220;biological mechanisms.&#8221; One of these mechanisms involves exosomes protecting and repairing pancreatic cells, particularly pancreatic &#946;-cells, which are responsible for insulin production. Damage or death of &#946;-cells is a primary cause of diabetes since, without these cells, the body cannot produce enough insulin.</p><p>Exosomes carry &#8220;packages&#8221; containing regenerative molecules or molecules that interact to inhibit certain proteins, such as p38, to prevent &#946;-cell apoptosis (cell death) and accelerate their repair, ensuring that the pancreas continues producing insulin normally. Additionally, the &#8220;communication&#8221; facilitated by exosomes enhances insulin sensitivity, allowing blood glucose to be absorbed quickly and preventing glucose buildup in the bloodstream.</p><p>Moreover, exosomes contribute to maintaining fat metabolism. The relationship between insulin sensitivity and fat metabolism is proportional&#8212;when insulin functions properly and its sensitivity increases, fat metabolism runs smoothly, preventing fat accumulation. Conversely, impaired fat metabolism leads to fat buildup in specific tissues, reducing insulin sensitivity and hindering fat distribution for energy production.</p><h3>The Future of Diabetes Treatment</h3><p>Compared to stem cell therapy&#8212;another regenerative treatment&#8212;exosomes are considered to have fewer side effects. Unlike stem cells, exosomes do not trigger immunogenic reactions (immune responses) or side effects such as tumor growth.
